Q: What types of services do you offer?
A: Community Physical Therapy provides care for a wide variety of orthopedic conditions relating to muscles, ligaments and joints. Community Physical Therapy also offers a balance program to prevent falls with those who have balance related issues. We also offer pediatric physical therapy for orthopedic related problems. In addition, CPT also performs functional capacity evaluations and individualized work conditioning programs.

Q: How long do I have to wait for an appointment?
A: CPT will offer you an appointment within 48 hours of your call into the clinic.

Q: What hours does CPT have available for patients?
A: CPT offers early morning, evening and Saturday appointments.

Q: Do I need a physician's referral to be seen by a Physical Therapist?
A: No. Illinois law permits evaluation by a Licensed Physical Therapist without a physician's referral. However, to continue with treatment, a referral is required from a physician, podiatrist, physician assistant or nurse practitioner.

Q: Does my insurance cover the cost of physical therapy?
A: Yes, in most cases. Upon calling in for your initial appointment, we will take down your insurance information and confirm your coverage. We will then call you to inform you of your coverage.

Q: Do you accept my insurance?
A: Community Physical Therapy is "in-network" for many insurance plans. You can check the insurance section of our web site or call 630-282-5588.

Q: Who will treat my condition?
A: You will be seen by a licensed physical therapist. In addition, you will be seen by the same therapist for each of your appointments.

Q: Do you accept workman's compensation?
A: Yes, we do accept workman's compensation. We will verify coverage with your workman's compensation carrier and bill them directly.

Q: Do you accept litigation cases?
A: No, you will be responsible for the bill at the time services are rendered.

Q: Do I need a referral for the Body Balance Golf Program?
A: No, only if you require physical therapy prior to beginning the program. However, it is always wise to have a physical by your physician prior to beginning any new exercise program.

Q: What benefits, as an employer, would I get by utilizing your services?
A: Community Physical Therapy offers initial appointments within 24-48 hours of the call into the clinic. Our average number of visits to treat most non-surgical orthopedic cases is 7. Our treatment emphasis is on patient education and self-management which allows the patient to respond quicker to the rehabilitation process and keeps the cost down.
